INSTRUCTIONS FOR OBTAINING A WORK PERMIT
The state of Ohio requires a work permit for all employed students under the age of 18 years.
- If you are employed in West Virginia (even though you are a St. Clairsville High School student), you must secure a work permit from the state of WV at the Ohio County School Board office in Elm Grove.
- The student needs to complete the Student/Applicant Information section.
- The parent/guardian must sign the Student/Applicant Information section.
- The employer needs to complete and sign the Pledge of Employer section.
- IF YOUR EMPLOYER REQUIRES YOU TO HAVE A PHYSICAL, have your doctor complete the physician’s form.
- The student must be present when the permit is issued and have identification.
- When the form is completed, please bring it to Joyce Zeiher in the High School Office. A work permit will be issued to be taken to the employer.
